Patients travelling outside Northern Ireland for treatment

If you wish to receive medical treatment outside Northern Ireland, it’s important to understand how it works and the assistance you may be able to access.

This guidance applies to patients who are travelling outside Northern Ireland to access healthcare assessment and/or treatment. It is not relevant for travel within Northern Ireland for health and social care services.

Practice Registration Application

This form should be completed by all new patients.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

If you do not have a current medical card with your last GPs details you need to complete one of the following forms.

HS200 Last GP NI

 If you have misplaced your valid NI Medical card or wishing to move GPs within Northern Ireland

HSCR1 This form should be completed by any new patient from outside Northern Ireland wishing to register at your practice to obtain free treatment. 

This includes:

 

  • Persons who are registered with a GP Practice in England, Scotland or Wales and are moving to Northern Ireland to reside or to stay here as a visitor.

 

  • Persons who have come to Northern Ireland from other EEA countries or overseas. This can be someone born here who moved away and has returned for a visit or to take up residence again, or someone who has never been in Northern Ireland before.


'Registration' With The practice

PreRegistrationIf you wish to register. Please contact the surgery   Tel 02890 608500. When you visit the surgery for the first time you will be asked to complete forms within our registration pack. 

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
